Candidate List of Registration requirements

1. CVWe need an up to date copy of your CV

2. REFERENCES We need details of 2 PROFESSIONAL REFERENCES which MUST COVER YOUR PREVIOUS 3 YEARS OF EMPLOYMENT

3. IDENTITY DOCUMENT Identity checks will confirm that you are who you say you are:

List 1: Examples of photographic personal identity documents you can provide. NOTE: All forms of documentary evidence must be original and, if dated, must be valid and current, as indicated in the recommended list of documents below. Examples of photographic personal identity documents include:

  • Full, signed UK (Channel Islands, Isle of Man or Irish) passport or other nationalities passport
  • UK Biometric Residence Permit (BRP) card
  • UK full or provisional photocard driving licence. (If issued by the DVLA you are not required to see the paper counterpart. Licences issued in Northern Ireland by the DVA must be presented with the paper counterpart)
  • Other nationalities photocard driving licence. (Valid up to 12 months from the date when the individual entered the UK. The person checking the document must be able to assure themselves that the licence is bona fide)
  • HM Armed Forces Identity card (UK)
  • Identity cards carrying the PASS (Proof of Age Standards Scheme) accreditation logo (issued in the UK, Channel Islands and Isle of Man only). Organisation identity cards are not acceptable as they do not contain watermarks, holograms or other security markings
  • EEA government issued identity cards that comply with Council Regulation (EC) No 2252/2004, containing a biometric

4. PROOF OF ADDRESS DOCUMENT Candidate must provide TWO proofs of address documents WHICH MUST BE DATED LESS THAN 3 MONTHS OLD of you joining Springday Healthcare

The documents can be:

  • Bank statement
  • Utility bill – gas or electricity bill
  • Mobile phone bill
  • Water bill
  • Council tax letter
  • Tenant letter
  • Tenancy agreement
  • University or college letter

You must provide TWO OF the above documents

5. COPY OF YOUR ENHANCED DBS CERTIFICATE/CRIMINAL RECORD CHECK Your DBS Certificate must be  on the DBS UPDATE SERVCE – if not you will have to undertake a DBS check with Springday Healthcare

6. OVERSEAS POLICE CHECK Overseas Police check is required if YOU have  been out of the country for 6 months in the last 5 years

7. CLINICAL CARE TRAINING CERTIFICATES Candidates should provide all copies of care training certificates undertaken:

CARE SUPPORT WORKERS TO PROVIDE

  • NVQ level 2/3 Health and social care training/certificates
  • Care Certificate

QUALIFIED NURSES TO PROVIDE

  • Copies of their nursing diploma
  • Training undertaken for any specialties

8. CSTF ALIGNED MANDATORY TRAINING CERTIFICATE CSFT MANDATORY ALIGNED TRAINING   – WE NEED Verified copies of original mandatory training certificates covering the duration of the placement. Training must be Core Skills Training Framework Aligned (CSTF aligned) . Must be renewed Annually

  • Practical Life Support Basic or advanced (adult / paediatric / neonatal as appropriate to role) and compliant with the UK Resuscitation Council Guidelines or European Resuscitation Council Guidelines.
  • CTG – Midwife only
  • Caldicott Principles / Information Governance
  • Complaints Handling
  • Food Hygiene
  • Infection prevention and control Level 2
  • Lone Worker
  • PMVA / Breakaway – (Nurses / HCA working in Mental Health)
  • Mental Capacity Act (working in Mental Health)
  • Mental Health Act (working in Mental Health)

9. OCCUPATIONAL HEALTH/ IMMUNISATIONS and FITNESS TO WORK CERTIFICATE

Please provide blood test results confirming immunity to the following:

  • Hepatitis B (showing titre levels >100iu/ml or indicate if non-responder to vaccine)
  • Measles antibodies
  • Mumps antibodies
  • Rubella antibodies (German Measles)
  • Varicella antibodies tested
  • Negative Tested for infection for HIV, or Hepatitis C (Exposure Prone areas only)
  • BCG (Tuberculosis Vaccination) (evidence of scar)
